Also, I see that this problem has some similarities to this other discussion, so I wonder if they are related?
In this other case I think the problem was that there were 2 tape copies with the same Archive File ID being written to the same tape file. Usually each copy should end in a different tape pool, which meand they would never end up in the same tape.
