Has anyone used haproxy or similar to apply load balancing to cta-frontend-grpc?
What are other solutions in the event cta-frontend dies?

I assume you are running dCache+CTA. If that’s correct, then we are in the process of updating the driver to support multiple frontends and pick the one that is alive. I hope that the new driver version will be available by the end of the week.
